29 CFR  1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  a) Falls River Health Services dba Seven Sisters Living Center at 1201 Highway 71 in Hot Springs, SD 57747: On or about and at times prior to October 13, 2021, the employer did not ensure all required information was completed in detail on OSHA Form 300 for 2021 in that  Section (E), Where the Event Occurred, does not identify the location within the long term care facility were incidents occurred.   b) Falls River Health Services dba Seven Sisters Living Center at 1201 Highway 71 in Hot Springs, SD 57747: On or about and at times prior to October 13, 2021, the employer did not ensure all required information was completed in detail on OSHA Form 300 for 2021 in that  Section (F) does not include the following information for each entry; a description of the  injury or illness, parts of the body that are affected, and the object/substance that directly injured or made person ill.   c) Falls River Health Services dba Seven Sisters Living Center at 1201 Highway 71 in Hot Springs, SD 57747: On or about and at times prior to October 13, 2021, the employer did not ensure all required information was completed in detail on OSHA Form 300 for 2021 in that  Section (K) and (L) include partial days for some incidents.   Abatement Note: COVID-19 can be a recordable illness if a worker is infected as a result of performing their work-related duties. However, employers are only responsible for recording cases of COVID-19 if all of the following are true:  1. The case is a confirmed case of COVID-19 (see CDC information on persons under investigation and presumptive positive and laboratory-confirmed cases of COVID-19); 2. The case is work-related (as defined by 29 CFR 1904.5); and 3. The case involves one or more of the general recording criteria set forth in 29 CFR 1904.7 (e.g., medical treatment beyond first aid, days away from work).  Employers should follow the OSHA enforcement guidance found in the Updated Interim Enforcement Response Plan for Coronavirus Disease 2019 (COVID-19).
