29 CFR  1926.451(c)(2)(v):  The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  a.  On or about October 18, 2021, at the above addressed jobsite, an employee, performing residential carpentry operations from a platform raised, approximately eleven (11) feet eight (8) inches above ground, by the forks of a JLG Telehandler, did not have the platform attached to the forks, thereby exposing the employee to fall hazards.

29 CFR  1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a.  On or about October 18, 2021, at the above addressed jobsite, an employee, performing residential carpentry operations from a platform raised, approximately eleven (11) feet eight (8) inches above ground, by the forks of a JLG Telehandler, was not utilizing a personal fall arrest system, thereby exposing the employee to fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about October 18, 2021, at the above addressed jobsite, an employee performing residential carpentry operations, eleven (11) feet four (4) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about October 18, 2021, at the above addressed jobsite, an employee performing residential carpentry operations, ten (10) feet two (2) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.
