The employer installed and maintained flammable storage of propane bottles and lockers containing Methyl Ethel Ketone, Isopropyl alcohol, directly at the exit door leading to the building exterior.  Violation observed on or about October 19, 2021

Employees engaged in the manufacture of resins worked from a company manufactured, personnel cage/basket attached to a Multiton SM20 Walker Stackie forklift, which was not properly documented as being authorized for use by the manufacturer for use on the equipment. Employees engaged in the manufacture of resins used Sigma Mixers which had inadequate guards installed that still allowed an employee to reach into the danger zone of the machine during operation.
