Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor an alternative fall protection measure that meets a provision described in paragraph (b) of this section. On or about October 19, 2021, at the construction site located at 5616 Castle Peak, Benbrook, Texas, employees engaged in framing work without fall protection were exposed to unprotected fall hazard(s) of approximately 10 feet to the ground below.

On or about October 19, 2021, at the construction site located at 5616 Castle Peak, Benbrook, Texas, the employer did not provide an extension ladder where employee(s) could access and egress the residential building second level, exposing them to potential fall hazard(s).
