29 CFR  1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.     On October 8, 2021, the employer did not provide an extension ladder where employees could access and egress the residential building second level, exposing them to potential fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).     On October 8, 2021, employees engaged in framing work without fall protection were exposed to unprotected fall hazards of approximately 10 feet to the ground below.   Ismael Hernandez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1488125, citation number 01, item number 001 and was affirmed as a final order on September 21, 2020, with respect to a workplace located at 880 Lake Carolyn Parkway, Irving, Texas 75039.
