29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that would not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a) Stonebridge Elementary School, 12750 Bedrock Blvd, Spanish Fort, Al.; On or about October 19, 2021: The employer exposed employees to fall hazards of 14 feet 6 inches to the ground below in that the employer failed to ensure an extension ladder which was not able to extend three feet above the landing surface was either secured at its top to a rigid support and/or a grasping device was installed to allow employees safe access on/off the landing area.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Stonebridge Elementary School, 12750 Bedrock Blvd, Spanish Fort, Al.; On or about October 19, 2021: The employer failed to ensure fall protection was used while sheathing a 6:12 pitch roof which exposed employees to the hazards associated with falls of 14'5" to the ground below.  James Connolly was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 1449413, citation 1, item number 1 and was affirmed as a final order on March 5, 2020, with respect to a workplace located at 11364 CR 64, Belforest Elementary School Bldg., E, Daphne, Alabama.  James Connolly was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 1389870, citation 1, item number 1 and was affirmed as a final order on May 2, 2019, with respect to a workplace located at Bay Minette Elementary, 800 Blackburn Avenue, Bay Minette, Alabama.
