29 CFR  1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:(a)(IDENTIFY LOCATION(S))(DESCRIBE CONDITIONS AND TYPE OF AERIAL LIFT)   On or about October 20, 2021 Employees were observed working off an aerial lift and forklift with out utilizing fall protection in the  following instances.   1) Employee working on aerial lift were observed not anchored in the basket.   2) Employee working on forklift/man-lift were observed not anchored in the basket.
