29 CFR  1926.451(c)(2)(i):  Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.  On or about 10/20/2021, Amaya Construction LLC was installing siding on a residence located at 113 Golden Gate Drive in Wentzville, MO.   Two employees were working from pump jack scaffolding, approximately twelve feet above the ground, on the side of the residence.  All three scaffolding poles were not on footings which were sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.

29 CFR  1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  On or about 10/20/2021, Amaya Construction LLC was installing siding on a residence located at 113 Golden Gate Drive in Wentzville, MO.  Two employees were working from pump jack scaffolding on the side of the residence, approximately twelve feet above the ground, without utilizing fall protection.

On or about 10/20/2021, Amaya Construction LLC was installing siding on a residence located at 113 Golden Gate Drive in Wentzville, MO.   Two employees were working from pump jack scaffolding on the side of the residence.    The pump jack scaffolding's three poles were not secured to the structure by rigid triangular bracing or equivalent means at the bottom.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On or about 10/20/2021, Amaya Construction LLC was installing siding on a residence located at 113 Golden Gate Drive in Wentzville, MO.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The crew lead was observed working on the 7/12 sloped roof, approximately twelve feet above the ground, without utilizing fall protection.
