(a)	The employer failed to provide certification certifying the abatement method and completion for an employee conducting roofing work from a ladder jack scaffold not protected from falling to lower level issued on 12-1-2021 as Citation 1 Item 1a.

(a)	The employer failed to provide documentation demonstrating the abatement method and completion for an employee conducting roofing work from a ladder jack scaffold with no training issued on 12-1-2021 as Citation 1 Item 1b.

29 CFR  1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a)  On or about, October 20, 2021, an employee conducting roofing work from a ladder jack scaffold was not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 15.2 feet.
