29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about October, 20, 2021, on the north side of the construction site, employees engaged in framing work without fall protection were exposed to unprotected fall hazards of approximately 20 feet to the ground below. Raul Mendez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1292995, citation number 01, item number 003, issued on 2/6/2018 and was affirmed as a final order on 2/18/2018, with respect to a workplace located at 1107 Knoll Creek Dr. Mansfield, Texas 76063.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 20, 2021, on the commercial construction site, employees using an extension cord to energize a Makita skill saw that had the outer sheathing pulled away exposing internal wiring were exposed to electrical shock hazard.
