29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: (a) On or about 10/7/2021 - at 3350 Morgan Drive, Vestavia Hills, AL 35216, in the passageway leaving the Bakery into the backroom, employees were exposed to trip and fall hazards in that the passageway was not kept clean and orderly.

When the direction of travel to the exit or exit discharge was not immediately apparent, signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. On or about 10/7/2021 and at times earlier - 3350 Morgan Drive, Vestavia Hills, AL 35216, at the left turn from the bakery egress route to the exit discharge, a left turn was required to continue to the exit discharge and signs were not posted along the exit access indicating the direction of travel to the nearest exit.

29 CFR 1910.37(b)(5):Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet): (a) On or about 10/7/2021 and at times earlier - 3350 Morgan Drive, Vestavia Hills, AL 35216, at the unmarked double door along the retail side emergency egress route at the head of aisle #4 between the refrigerators for bacon on the left and cheese and pasta on the right, employees were exposed to burn hazards from fire, and suffocation hazards from smoke because the doors were not marked by a sign reading "Not an Exit." or similar designation, or identified by a sign indicating its actual use. (b) On or about 10/7/2021 and at times earlier - 3350 Morgan Drive, Vestavia Hills, AL 35216, at the unmarked double door along the emergency egress route in the storage room that led to the head of aisle #4 between the refrigerators for bacon on the right and cheese and pasta on the left, employees were exposed to burn hazards from fire, and suffocation hazards from smoke because the doors were not marked by a sign reading "Not an Exit." or similar designation, or be identified by a sign indicating its actual use. (c) On or about 10/7/2021 and at times earlier - 3350 Morgan Drive, Vestavia Hills, AL 35216, at the door into the bakery, employees were exposed to burn hazards from fire, and suffocation hazards from smoke because the door was not marked by a sign reading "Not an Exit." or similar designation, or be identified by a sign indicating its actual use.
