29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet: Healthcare Services Group, Inc. worksite located at 4401 Haverford Avenue, Philadelphia, PA 19104: On or about September 9, 2022 the employer did not maintain a list of all hazardous chemicals in the workplace, including but not limited to: a) Diversey Glance Glass & Multi-Surface Cleaner, which contains hazardous chemicals such as sodium lauryl sulfate. b) Diversey Virex II 256 Disinfectant Cleaner & Deodorant, which contains hazardous chemicals such as didecyl dimethyl ammonium chloride, n-alkyl dimethyl benzyl ammonium chloride, and ethyl alcohol. c) Diversey Crew Disinfectant Cleaner, which contains hazardous chemicals such as alcohol ethoxylates and n-alkyl dimethyl benzyl ammonium chloride.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use: Healthcare Services Group, Inc. worksite located at 4401 Haverford Avenue, Philadelphia, PA 19104: On or about September 9, 2022 the employer did not have safety data sheets for all hazardous chemicals in the workplace, including but not limited to: a) Ecolab Tri-Star Laundry Detergent Plus, which contains hazardous chemicals such as sodium hydroxide. b) Earth Laboratories, Inc. Peroxy HDOX, which contains the hazardous chemical hydrogen peroxide.
