29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: MCCAUSLAND GREENHOUSE: On or about October 21, 2021, the employer had not established and implemented a site specific hazardous energy control lockout/tagout program consisting of machine specific step-by-step written procedures and employee training for the employees during servicing and maintenance of equipment, such as (but not limited to) main panel/manual switch for the Bay 18 east in the McCausland greenhouse.

29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: MCCAUSLAND GREENHOUSE: On/or about October 21, 2021, the hazardous energy control lockout/tagout procedures were not written in step-by-step machine specifics for the employees during servicing and maintenance of equipment, such as (but not limited to) main panel/manual switch for the Bay 18 east in the McCausland greenhouse. On this day, the shirt sleeve of an employee replacing a bolt in the drive shaft at Bay 18 East in the McCausland greenhouse was caught in the drive shaft when the energy source had been turned back onto automatic and fell approximately 18 feet resulting in broken bones.
