29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating: a) Machine shop: On or about October 21, 2021, and at times prior, employees were exposed to electric shock hazards, in that the employer did not replaced the on/off switch knob for press machine #2 (SEYI, model SN1-39) requiring employees to insert their fingers into the switch opening of the 480 volt electrical box to turn off the equipment during die changes.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) Machine shop: On or about October 21, 2021, and at times prior, employees were exposed to electric shock and trip hazards, in that the employer used flexible cords and cables as a substitute for fixed wiring to provide electrical power to machine press #2, SEYI, model SN1-39. b) Machine shop: On or about October 21, 2021, and at times prior, employees were exposed to electric shock and trip hazards, in that the employer used flexible cords and cables as a substitute for fixed wiring to provide electrical power to machine press #3, SEYI, model SN1-50. c) Machine shop: On or about October 21, 2021, and at times prior, employees were exposed to electric shock and trip hazards, in that the employer used flexible cords and cables as a substitute for fixed wiring to provide electrical power to machine press #4, SEYI, model SN1-66. d) Machine shop: On or about October 21, 2021, and at times prior, employees were exposed to electric shock and trip hazards, in that the employer used flexible cords and cables as a substitute for fixed wiring to provide electrical power to machine press #5, SEYI, model SN1-121.
