The employer did not verify the affected employee had received LOTO certification when assigned to conduct preventative maintenance of the Delivery Bar Code Sorter (DBCS) equipment located at 1720 Market Street, St. Louis, MO 63155.     The affected employee was assigned to perform preventative maintenance that included removing dust and replacing of rollers and belts on DBCS machines 18, 19, 20, and 21.
