29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards: A safety and health program with policy, hazard assessment, and work site rules was not developed and/or implemented at this job site. Employees were exposed to stuck-by, caught-between, and fall hazards. Employees engaged in millwright, installing steel fabrication activities were exposed to stuck-by, caught-between, and fall hazards while working near an unguarded elevated work platform, and working from heights greater than 6 feet above a lower level without guardrail. Employees were exposed in that the employer had not developed and maintained a comprehensive safety and health program which addressed these and other hazards.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: At the jobsite of National Beef Plant by the fabrication floor area of the 1/2 Chuck Upgrade Table, Fabrication mezzanine BT-4 located at 2000 E Trail St. Dodge City, KS 67801: Employees engaged in installing new catwalk and conveyor activities from an elevated platform were exposed to fall hazards of approximately 10 feet from the edge of the mezzanine to the 1/2 table below in that fall protection was not utilized. Handrails have been removed for the installation of the new catwalk.
