On or about October 22, 2021, a defective cord was in use on the food heater on the side opposite of the drive-thru preparation line, exposing employees to contact with electrical current.  The cord for the food heater was missing the equipment grounding conductor pin and showed a strain relief issue where the protective jacket had been pulled back from the conductors.
