29 CFR 1910.502(f)(2)(i): The employer did not ensure that a respirator was used in accordance with 29 CFR � 1910.134 when employees have exposure to a person with suspected or confirmed COVID-19. Lakefront Skilled Nursing Facility, LLC located at 7618 N. Sheridan Road in Chicago, Illinois, 60626: a) On or about October 1, 2021 through October 21, 20211, a housekeeping employee was required to use a tight-fitting filtering facepiece N95 respirator, Makrite Sekura-N95, when performing janitorial services in the Red Zone where confirmed positive COVID-19 residents were located in the facility. The housekeeper had not been medically evaluated and fit tested prior to the required respirator use, in accordance with the requirements of 29 CFR 1910.134. b) On or about October 1, 2021 through October 21, 2021, a Certified Nurse Assistant (CNA) was required to use a tight-fitting filtering facepiece N95 respirator, Makrite Sekura-N95, when providing care to confirmed positive COVID-19 residents in the Red Zone. The CNA had not been medically evaluated and fit tested prior to the required respirator use, in accordance with the requirements of 29 CFR 1910.134. c) On or about October 1, 2021 through October 21, 2021, a housekeeping employee was required to use a tight-fitting filtering facepiece N95 respirator, Makrite Sekura-N95, when performing janitorial services in the Red Zone where confirmed positive COVID-19 residents were located in the facility. The housekeeping employee was observed wearing a surgical mask underneath the N95 respirator, which interfered with the respirator seal. No abatement certification or documentation is required for this item.
