(a)	The employer failed to provide documentation demonstrating the abatement method and completion for employees performing roofing work on a two story single family residential home without any fall protection exposing employees to a fall hazard issued on March 25, 2022 as Citation 1 Item 2.    (b) The employer failed to provide documentation demonstrating the abatement method and completion for employees performing roofing work on the porch portion and main house portion of roof on a two story single family home without any fall protection issued on  March 25, 2022 as Citation 1 Item 3a and 3b.

(a) On or about October 20, 2021 at the job site located at W200N5271 Beechcraft Rd, Menomonee Falls WI 53051 employees were not wearing eye protection while using a pneumatic nail gun to install shingles on a roof.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about October 20, 2021, at the jobsite located at W200N5271 Beechcraft Rd, Menomonee Falls WI, employees were performing roofing work on a two story single family residential home without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about October 20, 2021, at the jobsite located at W200N5271 Beechcraft Rd, Menomonee Falls WI, an employee were  performing roofing work on the porch portion of roof on a two story single family home with a pitch of 5:12 and up to approximately 14 7/10 feet without any fall protection in place, thereby exposing the employee to a fall hazard.  (b) On or about October 20, 2021, at the jobsite located at W200N5271 Beechcraft Rd, Menomonee Falls WI, employees were performing roofing work on the porch portion of roof on a two story single family home with a pitch of14:12 and up to approximately 22 1/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.
