29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:   a) Job site, 2024 and 2026 Stonelea Street, Pittsburgh, PA: On or about October 20, 2021, during roofing work, the employer did not ensure that appropriate fall protection systems/devices were in use where employees were exposed to fall hazards of up to at least approximately 20 feet.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Job site, 2024 and 2026 Stonelea Street, Pittsburgh, PA: On or about October 20, 2021, during roofing work, the employer did not ensure that  the ladder used to gain access to/egress from the roof was extended at least 3 feet above the landing area, as it was only extended approximately 2 1/2 feet above the roofline.
