29 CFR 1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) ASI Saw: On or about October 22, 2021 and times prior, employees were exposed to flying wood chips and sawdust while using saws to cut lumber for roof trusses. The employees did not utilize eye protection while working with the saw or using compressed air to clean equipment.

The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures. a) Fairman Building Components: On or about October 22, 2021 and times prior, employees were not required to wear a substantial leather shoe while performing work in an industrial environment.

The man basket was built by the employer and has not had written approval from the manufacturer.

29 CFR 1910.178(q)(5):All parts of any such industrial truck requiring replacement shall be replaced only by parts equivalent as to safety with those used in the original design. a) Nissan F02 Forklift: On or about October 22, 2021 and times prior, the employer replaced the seat on a Nissan F02 forklift. The seat was not replaced with one of original design in that the replaced seat did not come equipped with a seatbelt.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a) Maintenance Bay: On October 22, 2021 and times prior, the employer did not ensure that oxygen cylinders were separated from acetylene cylinders by at least 20 feet or a 5 foot noncombustible barrier. An oxygen and acetylene cylinder were found strapped together against the wall.
