29 CFR 1910.38(c)(2): Employer did not follow their own written procedures for type of emergency evacuation. 721 BANK STREET, NEW LONDON, CT: On or about October 20, 2021, four employees became ill within two hours of arriving at work. Their symptoms included severe headaches, nausea, and vomiting. They reported their symptoms to their supervisor who, after consulting with the Director of Environment, Health, and Safety, told the employees to finish drawing bloods for the patients who were in the waiting room and process the specimens before leaving the building. The company's Facility Evacuation Plan identified situations where the building should be evacuated including, but not limited to "building air contamination".

29 CFR 1910.38(e): The employer did not designate and train employees to assist in a safe and orderly evacuation of other employees: 721 BANK STREET, NEW LONDON, CT: On or about October 20, 2021, employees had not been trained to assist in the safe evacuation of other employees in that none of the four employees at the above-referenced Patient Service Center knew the procedures to evacuate the building when they all became ill.
