On or about October 21, 2021, the employer did not ensure that each roofing employee working on a roof with unprotected sides and edges 6 feet more above lower levels were protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards approximately 32 feet to the ground below.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about October 21, 2021, and at times prior thereto, at a commercial construction job site, employees performing roofing work climbed an extension ladder to access and egress the work area. The ladder was not extended at least 3 feet above the upper landing surface exposing the employees to fall hazards of up to 18 feet.
