OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) On or about October 25, 2021, and at times prior, the employer did not ensure that employees were safe from crushing and struck-by hazards in that the Rotary Lift, model SPOA10N5G0, serial #11019228, located in bay 11, did not have the locking mechanism locked into place while the lift was raised and in use.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) On or about October 25, 2021, and at times prior, the employer did not maintain a clear distance of three feet in front of the circuit breaker panels order to provide access for emergency maintenance.
