29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) Mechanics Shop - On or about October 22, 2021 the employer allowed newly hired employee to perform task that exposed the employee to crush hazards while working on sanitation truck which required control of hazardous energy and the employer allow new hire employee to perform such work without applying a personal lock to the group lockout device.
