OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees due to exposure to struck-by hazards from falling material from the hoists:   a) On or before 10/26/2021 employees were exposed to a struck-by hazard from falling material while utilizing powered chain hoists to move parts at workstations throughout the facility due to the employer not conducting documented periodic inspection for the 18 hoist located at workstations on the shop floor.  b) On or before 10/26/2021, employees were exposed to a struck by hazard due to the safety latch located on hook being broken on the over head hoist ID# 002 that service the Puma SMX 2600ST CNC machine in the machine area. c) On or before 10/26/2021 employees were exposed to a struck by hazard due to hoist pendants not being legibly labeled for direction of movement in locations such as, but not limited to the hoist servicing the Shipping department 1/2 ton CM Vlustar LW-305LT and the hoist that services the Mazak Nexus 6000	 1/2 Ton Series 622 in the machine areas.
