The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to workplace violence hazards such as physical assault and homicide while performing security services at an indoor shopping mall, including approaching armed individuals to enforce a shopping mall's "Code of Conduct."  a) On October 25, 2021, and times prior thereto, contracted mall security officers patrolling the mall were exposed to verbal and physical assaults, including potential gun violence, when enforcing the mall's Code of Conduct which prohibited firearms on the premises.  Security officers approached and interacted with mall customers who were carrying firearms.  This included but was not limited to an individual who repeatedly and openly violated the property owner's prohibition of firearms in an argumentative and escalating nature.  Note) Certification and documentation of corrective action are required for this item.
