1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: On or about October 19, 2021, the employer did not ensure the procedure for controlling hazardous energy on Puller Units was utilized prior to changing Pads, resulting in the unexpected movement of the Puller Unit Machine while an employee was changing the #1 Pad on Puller Unit #2 on the North side of the Red Line.
