29 CFR 1910.95(g)(8)(ii)(B):Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: PRODUCTION AREAS: On or about April 21, 2022, after employees were identified as having standard threshold shifts, the employer did not provide them with refitting and retraining on the use of hearing protectors.

29 CFR 1904.10(a):The employer did not record all cases on the OSHA 300 Log when an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS. COMPANY-WIDE: On or about October 27, 2021, the employer had not recorded all employees whose annual audiograms indicated that they had experienced a Standard Threshold Shift in one or both ears.
