29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): On or about 10/27/2021 and at times prior, in the production center of the worksite located at 9101 NW 87 Avenue, Medley, Florida, 33178, the employer did not ensure an employee was removed from a caught-in hazard while performing trouble-shooting on the unguarded and energized conveyor gears on a Yalong Grinding machine & washer Model# SM2500 Serial# 2019053.
