29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 20,2021, the employer did not ensure employees, performing framing work activities, were in use of fall protection. This exposed the employees to a fall hazard greater than six (6 ) feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about October 20, 2021, the employer did not ensure the side rails of an extension ladder extended at least three(3) feet above the landing. This exposed the employees using the extension ladder to access the upper level of the building to a fall hazard.
