29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Employees were exposed to struck by hazards from forklifts. Forklift operators were not trained with training consisting of formal instruction, and practical training and evaluation. There was no written and practical evaluation of the forklift operator's driving.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Employees were exposed to struck by hazards as the Doosan 45 Propane forklift had a brake pedal without a nonskid cover and the backup alarm was not working. The Doosan forklift had not been taken out of service until it was in safe operating condition. b) Employees were exposed to struck by hazards as the Doosan Diesel forklift Driver's left light was not working. The Doosan forklift had not been taken out of service until it was in safe operating condition. c) Employees were exposed to struck by hazards as the Nissan Diesel forklift, the front lights were not working. The Nissan MWG forklift was not taken out of service until it was in safe operating condition.
