29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about October 27, 2021, employees conducting roofing work on a two story apartment complex were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18 feet.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about October 27, 2021, employees who were exposed to potential fall hazards while conducting roofing work on a two story apartment building had not been trained to recognize and minimize fall hazards associated with their specific work activities.

29 CFR 1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. (a) On or about October 27, 2021, employees were using an extension ladder to access a two story apartment building roof that had a broken rung. This exposed employees to a potential fall hazard of approximately 18 feet.
