29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: On or about October 27, 2021, tank/supply room of the concrete plant, the employer did not ensure that two electrical junction boxes had covers. The electrical junction boxes held electrical wire connections to power pumps and this exposed employees to electrical hazards.

This exposed the employees to crystalline silica.
