29 CFR  1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:   On or about 10/27/2021, at the jobsite located at 5750 Columbia Circle, West Palm Beach, FL 33407, employees were exposed to fire and explosion hazards while working in an area with explosive wood dust (i.e. Kst = 16.40 Bars Meters/second, Pressure ratio = 7.00) accumulated on ductwork, electrical equipment, masonry walls and floors.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) would be met:   On or about 10/27/2021, at the jobsite located at 5750 Columbia Circle, West Palm Beach, FL 33407, the employer did not develop a hazard communication program for protecting employees while using  hazardous materials on a daily basis, including but not limited to, Varathane Penetrating Wood Stain, Odorless Mineral Spirits, Varathane Ultimate Polyurethane Water Based, Valspar 2000, Rust-Oleum, BEHR Dynasty Enamel.

29 CFR  1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical:   On or about 10/27/2021, at the jobsite located at 5750 Columbia Circle, West Palm Beach, FL 33407, employees were exposed to a chemical hazard while working with hazardous chemicals for which a safety data sheet was not available such as, but was not limited to Varathane Penetrating Wood Stain, Odorless Mineral Spirits, Varathane Ultimate Polyurethane, Valspar 2000, Rust-Oleum, BEHR Dynasty Enamel.
