29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   On or about November 18, 2021, the employer failed to develop and implement an energy control program that includes training, written procedures, and audits to protect employees who perform maintenance and servicing such as, but not limited to,  blade changes on the Schelling FXH saw, and tooling changes, cleaning and adjustments on the Fulltek CNC machine and CMS CNC machine. Employees were exposed to hazards that may result in injuries such as  lacerations, amputations, and contusions.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about November 18, 2021, the employer did not ensure that machine guarding was provided on the CMS CNC machine, model: NC PF102 4T 4M, serial number: 1693 to prevent employees from accessing the cutting tools and areas in which they were exposed to struck by hazards created by the movement of the CNC table.   b) On or about November 18, 2021, the employer did not ensure that machine guarding was provided on the Fulltek CNC machine, model: CME-484, serial number: C10011 to prevent employees from accessing the cutting tools and areas in which they were exposed to struck by hazards caused by the movement of the gantry and rail system.

Employees were exposed to skin contact hazards, inhalation hazards, and fire hazards.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about October 27, 2021, the employer had not provided employees with information and training on hazardous chemicals such as, but not limited to dichloromethane, hydrogen, oxygen, propane, and carbon monoxide which are contained in SC-125 (adhesive) and compressed gas cylinders used by employees to bond acrylic parts, torch acrylic parts and to power a forklift.  Training should cover the written hazard communication program, health hazards associated with the chemicals, methods to detect the release of hazardous chemicals, measures employees can take to protect themselves from exposure to the chemicals, emergency procedures to follow during workplace incidents involving the hazardous chemicals and safety data sheets outlining the information included on a safety data sheet and where employees can access the safety data sheets in the workplace.
