29 CFR  1910.106(b)(2)(iv)(a):  Atmospheric storage tanks were not  adequately vented to prevent the development of vacuum or pressure sufficient to distort the roof of a cone roof tank or exceeding the design pressure in the case of other atmospheric tanks, as a result of filling or emptying, and atmospheric temperature changes:  Workplace, IMTT Chem South Area,  250 East 22nd St., Bayonne , NJ  a)  Atmospheric Storage Tank #14024 with a 154,178 gallon capacity was used it to store Furfuryl Alcohol for distribution.   The gooseneck atmospheric roof vent became clogged with a hard scale sludge as a result of product polymerization.  On 10-7-2021, after the tank had been emptied to 378 gallons and  with air temperatures at 80 degrees F,  the tank over-pressurized due to the clogged roof vent.  The over-pressure caused the tank to burst , ejecting the roof shell upward and igniting vapor resulting in fire/explosion.  Violation occurred on or about 10-6-21 to 10-7-21
