29 CFR 1910.176(b): Storage of material created a hazard. Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse. On or about October 27, 2021, racking systems used for storage of parts and materials, created a hazard in that the system was damaged in excess of allowable limits, exposing employees to a crushing hazard. Specifically, but not limited to the following damaged rack areas were identified: (a) Aisle 2-18-01-1 identified rack damage to horizontal beam with a deflection of three (3) inches. (b) Aisle 17-11-03-1 identified rack damage to horizontal beam with a deflection of one (1) inch. (c) Aisle 27-30-03-12 identified rack damage to horizontal beam with a deflection of 0.75 inch. (d) Aisle 29 rack upright was damaged with a deflection of 0.5 inch and 0.625 inch. (e) Aisle 90 rack upright was damaged with a deflection of 0.75 inch.

29 CFR 1910.178(h)(2): Where general lighting was less than 2 lumens per square foot, auxiliary directional lighting was not provided on the truck. (a) On or about October 28, 2021, lighting within the storage/racking system areas were not illuminated to provide at least 2 lumens and/or forklifts were not equipped with directional lighting.
