29 CFR  1910.67(c)(2)(v):  A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift:  On or about October 27, 2021, two employees installing fiber optic components were exposed to falling approximately 11 feet while working from two bucket trucks and personal fall protection systems were not attached to the boom or basket when in the elevated position.
