29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about October 26th, 2021, Elkhorn, Nebraska. An employee was observed working directly below multiple individuals on the roof. The employees on the roof were engaged in framing activities that involved construction material and construction tools.

An employee was engaged in residential framing activities using a pneumatic nail gun without using eye protection. This was most recently documented on or about October 26th, 2021, in Elkhorn, Nebraska.

29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  The employer is failing to protect employees from fall hazards. This was most recently documented on or about October 26th, 2021 in Elkhorn, Nebraska. Upon entry to the residential structure, a hole was located on the main level of the residential structure. The hole was not protected by a guardrail or other fall protection system.

The employer is failing to protect employees from fall hazards. This was most recently documented on or about October 26th, 2021 in Omaha, Nebraska. Two employees were observed working on a residential structure conducting framing operations. The exposed employees were standing on top of the top plates/trusses of the residential structure at heights over 6 feet- No fall protection was being utilized.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing Employer is failing to protect employees from fall hazards associated with framing operations on new residential structure located in  Elkhorn, NE 68022; An extension ladder was used for access and egress to the upper level of the residential structure and it did not extend at least 3 feet above the surface being accessed. Also, the ladder was also not secured at its top to a rigid support.
