29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were maintained in a legible condition: Employees engaged in material handing activities using the Tusk forklift #8849J, serial number 7081, model number 2500PD-8 were exposed to struck-by hazards in that the following markings were not in place and legible: a) The control lever that raises and lowers the mast (up and down) was not marked; and b) The control lever that tilts the mast (forward and back) did not include markings.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in drilling activities in the wood shop at the Grizzly 12-Speed Heavy-Duty 20" Floor Drill Press Machine, serial number 17363, model number G7948 were exposed to struck-by and amputation hazards in that machine guarding was not provided. The rotating drill chuck was not guarded, employees were not provided protection from flying chips and debris.
