The employee operating the sheeting machine was caught in the point of operation, resulting in injuries to the employee's hand.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Main production area, 1075 Cambridge Street, Cambridge, MA a) On or about 10/23/21, the employer did not establish an energy control program that outlined procedures and training for employees conducting cleaning of a Jagum sheeting machine that was involved in an injury. Two employees worked together to sanitize the machine, which required disassembly.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department, or other group: Main production area, 1075 Cambridge Street, Cambridge, MA a) On or about 10/23/21, employees conducting cleaning tasks on the Jagum sheeting machine did not utilize a procedure prior to and while conducting cleaning/sanitizing of the machine, exposing employees to amputation and other serious injuries. The Jagum sheeting machine is cord and plug, however, two employees conducted work on the machine to sanitize.
