The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) On or about October 28, 2021, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 25417 Halburton, Beachwood, Ohio 44122. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.
