29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: Roof of Building #6. On or about October 27, 2021, an employee was exposed to struck-by injuries while using a DeWalt roofing nail gun, Model DW45RN, to attach asphalt shingles, without using eye/face protection.

29 CFR 1926.503(a)(2)(ii): The employer did not ensure that the employee was trained, as necessary, by a competent person qualified in the correct procedures for maintaining and inspecting the fall protection systems that were used: Roof of Building #6. On or about October 27, 2021, and at times prior thereto, an employee was exposed to a fall of greater than 6 feet to the ground below, while installing asphalt shingles on a roof without being trained to maintaining and inspecting the personal fall arrest system lanyard prior to use.
