29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 18, 2021 at 4765 Earth City Expressway, Saint Louis, Bridgeton, Missouri 63044, Veripac LLC had not ensured that ingoing nip points for the "Litai Machinery" (Model No. TJ-750, Serial No. PLUSC20190725) Plastic Extruder were guarded. Employee(s) were exposed to ingoing nip points when the machine is in operation.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. At 4765 Earth City Expy, Bridgeton, MO 63044 - on or about 10/20/2021, Veripac, LLC reported an in-patient hospitalization of an employee. Veripac, LLC did not report the in-patient hospitalization within twenty-four (24) hours of the hospitalization.
