29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a.  On or about October 27, 2021, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about October 27, 2021, at the above addressed jobsite, an employee performing residential roofing operations, twenty-four (24) feet  five (5) inches above the ground was not protected from falls, thereby exposing the employee to fall hazards.  b. On or about October 27, 2021, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet  eight (8) inches above the ground were not protected from falls, thereby exposing the employees to fall hazards.
