29 CFR  1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a. At a newly constructed residential home:  On or about October 27, 2021 and at times prior to, employees were exposed to fall hazards, in that the employer failed to conduct regular and frequent inspections of the jobsite.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   a. At the front roof line of a newly constructed residential home:  On or about October 27, 2021 and at times prior to, the employer failed to protect employees from a 10.3-foot fall hazard in that there was no conventional fall protection being used by employees.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a. At the front roof line of a newly constructed residential home:  On or about October 27, 2021 and at times prior to, the employer failed to protect employees from a 10.3-foot fall hazard in that the portable extension ladder being used to gain access and egress to perform roofing work, did not have its side rails extended at least 3 feet above the upper landing surface of the roof.  b. At the side garage roof line of a newly constructed residential home:  On or about October 27, 2021 and at times prior to, the employer failed to protect employees from a 10.3-foot fall hazard in that the portable extension ladder being used to gain access and egress to perform roofing work, did not have its side rails extended at least 3 feet above the upper landing surface of the roof.
