(a) On or about October 28, 2021, at the jobsite located at 2000 American Eagle Way, Slinger WI 53086, employees were performing roofing work on a two story multi family apartment unit with a pitch of 7:12 and up to approximately 19 6/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about October 28, 2021, at the jobsite located at 2000 American Eagle Way, Slinger WI 53086, employees were performing roofing work on a two story multi family apartment unit with a pitch of 7:12 and up to approximately 19 6/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.
