29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  (a) Worksite - On or about October 29, 2021, the employer did not ensure that safe means of scaffold access and egress were available to  employees while laying bricks from a supported scaffold. Employees climbed a folded 6-feet stepladder to access the platforms that were approximately 15 ft. high above the ground.

Employees were working in scaffold with missing components such as guardrail systems and fully planked.

(a) Worksite - On or about October 29, 2021, a qualified competent person did not supervise the erection and movement of scaffold systems to assure that it was completely erected with all their components, to prevent fall hazards.

(a) Worksite - On or about October 29, 2021, employees were working from a scaffold platform that was 15 feet high above the ground without any fall protection or guardrail system installed.

(a) Worksite - On or about October 29, 2021, employees used a 6-feet stepladder, designed to be used in the open and braced position, was left in closed position while employees utilized it to climb over to gain access into a scaffold system from the residential structure under construction.

29 CFR  1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  (a) Worksite-  On or about October 29, 2021, employees were carrying pails full of mortar mix and other masonry materials while climbing up or down the stepladder that could have caused the employee to lose balance and fall to the ground.
