29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) New residential construction located on Lot #41 at 595 Martha Way, Alpharetta, GA 30009, Roof: On or about 10/15/2021, the employer exposed employees to fall hazards of 21-feet, 4-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in roof truss framing and sheathing activities using the 6/12 pitched roof as a walking/working surface.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) New residential construction located on Lot #41 at 595 Martha Way, Alpharetta, GA 30009, Roof - On or about 10/15/2021, the employer exposed employees to fall hazards of 11-feet, 7-inches to the lower level, in that the ladder side rails of the ladder provided were not extended at least 3-feet past the landing and the ladder was not secured to prevent accidental movement during use.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) New residential construction located on Lot #41 at 595 Martha Way, Alpharetta, GA 30009, Garage and driveway areas: On or about 10/15/2021, the employer failed to inspect all of the electrical cords and electrical powered equipment prior to the start of the work shift. The strain relief on two extension cords where damaged, missing ground pins on extension cords and cords powering the air compressor, and extension cords with damaged protective casings on the 50-ft. extension cord plugged into the neighboring structure.
